WestCare Lead Veteran Advocate in Fresno, California

Job Description:

Position Summary: Person in this position will assist in day-to-day operations of the program. This person is responsible for case management activities including linking veterans with services inside the Veterans Administration and in the community; case record management, and supervises Veteran Advocates, as assigned. Essential Job Functions include those listed below. Other duties may be assigned as needed.

Essential Job Functions:

  • Oversight of day-to day operations of the program;

  • Oversight of Veteran Advocates assigned to the program;

  • Support an active caseload of homeless veterans and/or veterans that are in danger of becoming homeless;

  • Participate in outreach efforts, including street outreach.

  • Intake and assessment of incoming veterans to determine appropriate services;

  • Make referrals and/appointments with appropriate agency, services and/or case managers;

  • Maintain client files according to company policy and contract requirements;

  • Heavy documentation and reporting;

  • Provide daily, weekly and monthly reports as requested;

  • Assist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested;

  • Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested;

  • Coordination of various services throughout several agencies;

  • Conduct outreach activities in the community including street outreach;

  • Attend scheduled meetings and presentations with referral agencies; and

  • Perform any other duties as assigned.

Essential Qualifications:

  • Excellent communication and public speaking skills;

  • Veteran and/or military experience preferred

  • Two (2) years case management experience;

  • One (1) Year supervision experience;

  • Good computer skills;

  • Good documentation skills;

  • Be able to multi-task and make independent decisions; and

  • CPR Certification, First Aid Certification and an Annual Tuberculosis test.


  • AA Degree is required; and

  • Graduation from an accredited four-year college with a degree in Psychology, Social Work, Rehabilitation Counseling or a related field is preferred.
