Community Subacute & Transitional Care Center

Community Subacute & Transitional Care Centeris a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision. CSTCC has two very unique and distinct units.

When a patient needs ongoing care, subacute care steps in. Our 58-bed subacute unit is medically certified as a rehabilitation respiratory unit. We specialize in trach weaning, helping patients come off of vents, feeding tubes, retraining patients to eat and breathe. The average length of stay for patients in subacute is about 6 months to one year.

Our 48-bed short-term skilled unit provides wound care, IV therapy, and focuses on getting the patients transitioned back to their lives and community. This unit houses a full rehabilitation gym for our patients.

Patients whose care requires ventilator, respiratory, wound, IV, hospice, rehabilitation services and/or general nursing can receive treatment that’s consistent with an acute care center’s effectiveness while in a comfortable long-term care setting.

Residents can expect the specialized staff of the Subacute & Transitional Care Center to assess and meet their individual needs so their personal focus is on rest and recovery.

Community Subacute & Transitional Care Center is seeking nurses like you for challenging and rewarding 8 and 12 hour shifts.

Responsibilities:

Assesses, plans and implements the nursing care of residents within the skilled nursing setting. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs and supportive staff members. Participates in resident and family teaching. Maintains role as resident advocate with a focus on the facility's mission.

Qualifications:

High School Diploma/GED. Fundamental acute-care nursing skills gained by completion of an accredited Vocational Nursing School program. Satisfactory completion of an approved Pharmacology course is required if licensed by waiver. PREFERRED: Six months experience in a long term care facility. LICENSES/CERTIFICATIONS MINIMUM REQUIRED: Current licensure as an LVN in the state of California. Current CPR card.
