WestCare Outreach Worker in Fresno, California
Essential Job Functions:
Serve as an active part of the FMCoC outreach team and carry out all required responsibilities as needed.
Provide screening, assessment and brief interventions to the homeless and those at risk of becoming homeless.
Provide navigation through referrals to community partners, mental health, case management services, education, and appropriate community resources;
Participation in facilitating ancillary services, i.e. continuing care, networking with community agencies, participating in Homeless Connect events and developing and updating referral and resource information, etc.;
Participation in case staffing, team meetings and Fresno Madera Continuum of Care Outreach Committee meetings as required.
Coordinate and partner with community resources to promote the wellbeing of the homeless and (i.e. WestCare, Fresno Housing Authority, Fresno County Department of Social Services, , VA, Vet Center, FMCoC, MAP at the Pov) Develop and maintain working knowledge of available community resources and act as liaison.
Conduct outreach activities in the City Fresno including street outreach, engagement, assessment, and navigation;
Attend scheduled meetings within the community;
Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
Perform any other duties as assigned.
Experience consisting of two (2) years of providing direct outreach services in an intensive setting;
Ability to work flexible schedule (schedule may include weekends and evening hours);
Veteran experience preferred
Must have the ability to pass a criminal history background screening;
Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required;
CPR Certification, First Aid Certification and an Annual Tuberculosis test.
Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or a related field is preferred;
GED or High School Diploma is required.
Work is primarily performed in an office or treatment facility setting; and
Some outdoor activities may be required.
The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee engaged in Category II tasks;
Occasional lifting of more than 10 pounds;
Transportation of clients in company vehicle;
Occasional risk of intensive physical activity with clients; and
Sitting, standing, walking, reaching are performed in the normal course of the position.
