WestCare Outreach Worker in Fresno, California

Job Description:

Essential Job Functions:

  • Serve as an active part of the FMCoC outreach team and carry out all required responsibilities as needed.

  • Provide screening, assessment and brief interventions to the homeless and those at risk of becoming homeless.

  • Provide navigation through referrals to community partners, mental health, case management services, education, and appropriate community resources;

  • Participation in facilitating ancillary services, i.e. continuing care, networking with community agencies, participating in Homeless Connect events and developing and updating referral and resource information, etc.;

  • Participation in case staffing, team meetings and Fresno Madera Continuum of Care Outreach Committee meetings as required.

  • Coordinate and partner with community resources to promote the wellbeing of the homeless and (i.e. WestCare, Fresno Housing Authority, Fresno County Department of Social Services, , VA, Vet Center, FMCoC, MAP at the Pov) Develop and maintain working knowledge of available community resources and act as liaison.

  • Conduct outreach activities in the City Fresno including street outreach, engagement, assessment, and navigation;

  • Attend scheduled meetings within the community;

  • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and

  • Perform any other duties as assigned.

Essential Qualifications:

  • Experience consisting of two (2) years of providing direct outreach services in an intensive setting;

  • Ability to work flexible schedule (schedule may include weekends and evening hours);

  • Veteran experience preferred

  • Bilingual preferred

  • Must have the ability to pass a criminal history background screening;

  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required;

  • CPR Certification, First Aid Certification and an Annual Tuberculosis test.

Education:

  • Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or a related field is preferred;

  • GED or High School Diploma is required.

Working Conditions:

  • Work is primarily performed in an office or treatment facility setting; and

  • Some outdoor activities may be required.

Physical Demands:

  • The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee engaged in Category II tasks;

  • Occasional lifting of more than 10 pounds;

  • Transportation of clients in company vehicle;

  • Occasional risk of intensive physical activity with clients; and

  • Sitting, standing, walking, reaching are performed in the normal course of the position.
