As a Patient Care Coordinator at AseraCare Hospice, you'll be responsible for daily management of nursing and patient care. Rather than treating the disease, you'll focus your staff's efforts on the whole person. With so many relying on your clinical leadership, direction and supervision, you'll help our patients live life to the fullest. You'll be an advocate for patients and families, maintaining existing programs and creating new ones to meet their special needs. Provide invaluable support for our patients and families during these difficult but significant moments in their lives as a Patient Care Coordinator by performing a wide variety of daily responsibilities.

GENERAL PURPOSE

Responsible for the day-to-day management of patient care activities and supervision of clinical staff. Serves as a consultant to hospice staff. Participates in quality improvement program and the evaluation of nursing and other clinical services. Supports the Provider Relations and volunteer activities of the organization.

ESSENTIAL JOB DUTIES

  • Supervises patient care team in the delivery of patient/family care. Supervises and assesses the quality of contracted services in relation to patient care. Receives and assigns case referrals to personnel

  • Assist director of clinical services in ensuring that all patient/family services comply with federal, state(s) and local regulatory requirements and with accepted professional standards

  • Participates in the development, implementation and evaluation of policies and procedures for patient/family services. Participates in the selection and evaluation of all direct patient/family care staff. Participates in the orientation and ongoing education of staff and volunteers

  • Coordinates the development, implementation and review of the interdisciplinary plan of care for each patient/family. Communicates as needed with all members of the team, the patient/family, the medical director, the administrator, and others involved in the care of the patient to facilitate optimal care of patients/families. Initiates preventive and rehabilitative nursing procedures. Reports changes in the patient's medical and mental condition to the attending physician as well as to other hospice staff and contract employees involved in the patient's car

  • Maintains the clinical records, including nursing clinical notes and progress notes, for each service provided to each patient according to established standards and agency policy. Assists with the processing and maintenance of all reports required for third-party billing. Conducts quarterly record reviews and communicates findings and recommendations to director of clinical services and hospice personnel

  • Assures the 24-hour availability of nursing services to patient/families and assists director of clinical services in assignment of nursing personnel. Participates in twenty-four hour, seven days per week call, as scheduled by the DOCS. Follow through with specific plan of care as established and modified by the interdisciplinary team with his/her individual comments and suggestions incorporated

  • Teaches patient and family members regarding nursing care skills, procedures, techniques and use of equipment

  • Participates in quality improvement reviews especially related to the evaluation of nursing and contracted services. Make recommendations for improving and enhancing the overall functioning of the agency

  • Manages the development of a discharge plan when a patient is discharged prior to death or transferred to another hospice

  • Participates in public relations and community activities that promote the organization's role as an effective member of the hospice system. Promotes customer service orientation to all personnel

  • Participates on committees and special projects and other duties as assigned. Contributes to team effort by accomplishing related results as needed. From time to time this position will be expected to perform on-call professional duties

  • Hospice personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs

Position Requirements

QUALIFICATIONS

  • Must be a registered nurse with a current state license

  • A nurse supervisor shall have a current RN license, with at least two (2) years of hospice, home health, or community health experience in the last five (5) years.

  • Must demonstrate a proven competency in all aspects of IV therapy

  • Must have a minimum of 2 years experience as staff nurse in an acute care hospital or home health agency. Experience in dealing with dying patients and their families with a good understanding of pain/symptom management

  • Must have a minimum of one-year of supervisory experience

  • Must be skilled in nursing practices, able to cope with family emotional stress, and must be tolerant of individual lifestyles

  • Must have 24-hour access to a reliable motor vehicle and must maintain a valid driver's license and maintain automobile insurance that complies with state and company regulations

  • Must be capable of maintaining regular attendance

KNOWLEDGE, SKILLS, ABILITIES & BEHAVIORS

  • Must possess intermediate level skills to assess and respond to the needs of patients and families in a home setting

  • Must be able to maintain confidentiality regarding patient and company proprietary information

  • Must have the ability to relate professionally and positively and work cooperatively with other employees at all levels

  • Must possess computer skills of the types and at a level necessary to accomplish the job

PHYSICAL AND SENSORY REQUIREMENTS

Mobility, reaching, bending, lifting (minimum of 10 pounds), fine hand coordination, ability to read and write, the ability to understand and follow written and oral instructions and directions, ability to travel, and ability to remain calm under pressure
